Remote Patient Monitoring & AI Clinical Intelligence

Attention that starts before a crisis, not after one.

Most falls, infections and hospital readmissions don't come out of nowhere — there are usually small, early signals. Our Remote Patient Monitoring (RPM) layer is built to help our nursing team notice those signals sooner, without turning your home into a hospital ward.

1

Gentle, everyday readings

With consent, simple home devices and visit observations capture things like blood pressure, pulse, mobility and general wellbeing — nothing invasive, nothing constant.

2

NEWS2-calibrated scoring

Readings are scored using an approach calibrated against the National Early Warning Score (NEWS2) methodology used across the HSE — a consistent, evidence-based way to gauge how urgent a change might be.

3

AI-assisted pattern spotting

Software flags meaningful changes from a person's own baseline — a gradual drift, not just a single odd reading — surfacing it for a human to look at, rather than acting on its own.

4

Always reviewed by a nurse

Every flag is triaged by our clinical team. If it needs attention, we act — a call, an earlier visit, or contacting your GP or the PHN — and you're kept informed.

RPM is a monitoring aid, not a diagnostic or emergency device. It does not replace 999/112, your GP, or clinical judgement — it simply helps our nurses prioritise their attention sooner.

Where technology meets human care

We believe everyone deserves to age with dignity, safely, in their own home. SolidRock exists to close the gap between warm human care and modern technology.

Independence, togetherness, and a little help when it's needed. That's the everyday moment SolidRock is built around.

"I've spent my career at the bedside — in ICU, in elder care, across two countries. I started SolidRock because families deserve both a carer who knows them by name, and the quiet reassurance of knowing how their loved one's day really went."

Frequently Asked Questions

Common questions, answered honestly

Care can be funded through the HSE Home Support Service, privately, or a combination of both. We'll help you understand what applies to your situation at your free assessment.
Yes. We match carefully for continuity, but if it's not the right fit for any reason, tell your coordinator and we'll arrange a change — no awkward conversations needed.
Yes, with reasonable notice as set out in your care agreement. We'll always talk it through with you first — there's no pressure to commit long-term.
After your free assessment, most care plans can begin within a few days, depending on the hours and skills required. For urgent post-hospital discharge, we prioritise a rapid response — let us know it's time-sensitive when you get in touch.
Yes. Care doesn't stop for holidays, and neither do we. We plan carer cover in advance for bank holidays and the festive period so your routine stays consistent.
Yes. Needs change, and your plan should too. Regular reviews mean we can scale hours up after a hospital stay, or down as confidence and independence return — just talk to your coordinator.
HSE-funded hours are allocated after a needs assessment by your local HSE team, at no direct cost to you. Privately-funded hours are arranged and paid for directly with us, and can top up HSE hours or stand alone. Many families use a mix of both.
Fair Deal (the Nursing Homes Support Scheme) is primarily designed for long-term residential care rather than home support, but it's often part of the wider conversation families have when weighing up options. We're happy to talk through where it fits, if at all, for your situation.
No — it's an indicative range only, to help with early planning. Your exact price depends on your care plan, agreed at your free, no-obligation assessment.
We follow a clear safeguarding policy with clinical oversight. Any concern is escalated promptly and handled with care, transparency, and in line with our statutory obligations.
Every carer is Garda-vetted, reference-checked, and right-to-work verified before their first visit, then completes structured induction training with ongoing supervision and CPD.
Matching considers care needs and skills, personality and interests, language, and practical factors like location and schedule. We aim for a small, consistent team around each client rather than a large rotating roster.
For any medical emergency, always call 999 or 112 first. Outside of scheduled visits, our office and on-call clinical team remain reachable for non-emergency concerns — we'll confirm exact out-of-hours arrangements as part of your care plan.
Only people your family member (or their legal representative) consents to. Access is permission-based and can be changed at any time — see our Privacy Policy for full detail.
RPM is gentle, consent-led tracking of things like vital signs and mobility trends, reviewed by our nursing team to catch changes early. It's entirely opt-in — some families want the extra reassurance, others prefer visits alone, and either is completely fine.
Yes — this is one of the most common reasons families choose us. With consent, the Family Portal can be shared with relatives anywhere in the world, so distance doesn't mean being out of the loop.
Data is minimised, encrypted, and handled under GDPR — we only collect what's needed to deliver safe care, and never sell personal data. Full detail is in our Privacy Policy, including your rights and how to exercise them.
If this is a medical emergency, please call 999 or 112 immediately. For urgent (non-emergency) health concerns, contact your GP or Public Health Nurse. SolidRock's website and AI Care Assistant are not medical advice services.
